Pay Range: $74,000-$84,000

Sign-On Bonus: $3,000.00

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time RN for our Community Support Program (CSP). CSP nurses work as a part of an interdisciplinary treatment team providing services to adults with severe and persistent mental illness.

Responsibilities

• Conduct comprehensive nursing assessments as part of a multi-disciplinary treatment team – initial health screenings, follow-up nursing appointments, various testing
• Consults with Riverbend providers, team members, family members, significant others, and community providers.
• Maintain your own caseload and cover other caseloads as needed
• Medication management – provide client education, administer medication (long acting injectable), refill management, prior authorizations, consults with pharmacies
• Attends treatment team meetings and other meetings as required
• Collaborate with Riverbend Integrated Center for Health (embedded PCP practice)
• Utilize electronic medical record (EHR) to document patient information, referrals or other relevant information as required.
• Adhere to all compliance procedures relevant to protected health information (PHI) and HIPAA regulations.

Requirements

• Valid New Hampshire Registered Nurse License
• At least one-year nursing experience required
• Certification in Basic Life Support
• Experience working with individuals with psychiatric illnesses preferred
• ANCC board certification in Psychiatric Mental Health Nursing preferred
• Valid driver’s license and personal vehicle or access to a reasonable alternative is preferred
