Job Summary

This posting is for (1) 0.60 FTE BSW Care Manager position at the University of Michigan Hospital, Cardiovascular Center, and Mott and Von Voigtlander Women’s Hospital in the Department. This position provides focused assessment of Social Determinants of Health for all patients admitted to inpatient status at Michigan Medicine. The schedule for this position is 3 days per week.  Weekend Coverage may be required to meet department and patient care needs at the discretion of the Care Management Department

Responsibilities*

Care Management BSW Services include but are not limited to the following: proactive screening of patients for Social Determinant of Health (SDOH) Vulnerabilities on identified patients to assess environmental needs and any barriers to achieving optimal health status, collaborate with multidisciplinary team and MSW Clinicians to provide resources, referrals, and interventions to address identified SDOH vulnerabilities. Care Management BSW’s aid in expediting discharge and support to inpatient MSW and RN Care Managers through proactive assessment of patient’s psycho-social and tangible needs. This position also supports regulatory requirements in the acute care setting specific to the required completion of SDOH assessments and interventions for admitted patients.

Required Qualifications*

Bachelor’s degree in Social Work from a school accredited by the Council of Social Work Education is necessary. Must have current clinical license (LLBSW or LBSW) to practice social work in the State of Michigan. Clinical practice concentration and field practicum or recent work experience in a community or health related agency, or equivalent experience necessary.

It is essential that the candidate be able to manage a large caseload with multiple competing demands. The ability to work independently in an emergency department setting. The successful candidate must also be able to work with multidisciplinary teams and demonstrate clear and timely documentation.

Desired Qualifications*

The ideal candidate will have medical social work experience, strong assessment skills, the ability to establish therapeutic relationships quickly, and to relate well to a diverse population. Interpersonal skills necessary to obtain information from and communicate effectively with patients, other Michigan Medicine staff, and community agencies or providers. Ability to establish patient relationships effectively and quickly. Analytical skills necessary in order to assess patients’ needs, identify and connect patients with appropriate multidisciplinary care providers, and provide brief support when indicated. Excellent written and verbal communication skills.

This position is included in the bargaining unit represented by the United Michigan Medicine Allied Professionals (UMMAP), which represents bargaining unit members in all matters with respect to wages, benefits, hours, and other terms and conditions of employment.

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings.  Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
