Your Role
The Care Management team will serve to support the mission of the department, which is to provide support to members in maintaining health and wellness in the outpatient setting. The supervisor of Care Management will report to the Senior Manager of Care Management. In this role you will lead the outreach and engagement program to identify members who would benefit from care management services and ensure that members are referred to the appropriate programs based on their individual needs. You will oversee the coordination of care for members, ensuring all services are delivered seamlessly and effectively. Your Knowledge and Experience
- Requires a minimum of 5 years of prior relevant experience
- Requires a minimum of 2 years of experience operating in a lead role or equivalent leadership training or manager training
- Experience working with the Commercial and Medicare population and knowledge of managed care highly desirable
- Detail-oriented with strong organizational and time management skills.
- Proficiency in data analysis and interpretation to guide decision-making and process improvements.
Pay Range
The pay range for this role is: $ 65890.00 to $ 98890.00 for California.
